About Us

Housing stabilization services is a new service that is funded by Medical Assistance health insurance through the state of Minnesota. This service helps people find and sustain housing. Those who have case management. Cadi Waivered and non-waivered Individuals who are 18 years of age and older and are struggle with a mental or physical disabilities, substance abuse disorder as well as individuals who are 65 older and on medical assistance are able to utilize this service and are encouraged to apply.

History

Housing Stabilization Services launched in 2020. This services has helped house many individuals who have experienced homelessness, those who were at risk of becoming homeless or those transitioning from a facility/institution into the community/independent housing. Housing Stabilization services is here to stay and is looking to help those in need for generations to come. However the goal is to help house individuals and keep them housed so that the trauma of being homeless can become a thing of the past.

Housing Stabilization Services (Transitional)

With HSS we offer Transitional Services. This is our main Housing service we offer while working with our clients. This services allows our staff to help Find, obtain and organize a move on behalf of our clients.

Housing Stabilization Services (Sustaining)

Housing Sustaining Services is our second service we offer. This service allows for the clients to have assistance from a worker through out many years to ensure that their housing needs are met and they do not run into unstable housing issues again.

We Assist With The Following:

• Keeping You Housed / Assist With Getting You Housed.
• Finding Funding for your New Housing, or Past Due Rent.
• Finding Shelter if Currently Homeless and Creating a Long-Term Stability Action Plan.
• Searching for and Applying for Housing that is Affordable and in Your Chosen Location.
• Understanding the Rental Process and Your New Lease Before You Sign.
• Creating a Budget Plan with You.
• Finding Funding for Deposits and Moving Costs.
• Request Transitional Services for People with Waivers Whom We Are Working With in HSS.
• Request Updated HFPCPs to Reflect Moving Expenses for Those Who Are Moving from a Medically Funded Facility or Are Currently Homeless and have stayed in a shelter at lease one night in the last 12 months and Are Not Waived.
• Understanding and Applying for Disability Services (if applicable).
• Understanding and Applying for Other Financial Assistance Programs.
• Navigating Housing Issues with Management and Other Residents.

Who Is Eligible for Housing Stabilization Services?

Qualification Requirements:
1. An individual with a disability.
2. Medical Assistance & 18+
3. Housing Instability, or are homeless,

Are you living situation that is provider-led and you want to move into a more independent setting.
Are you an Adults who have a waiver such as CADI, DD, BI, EW, or CAC waiver.
We also help Adults who do not have a waiver be approved as non-waivered once the following steps are completed: Complete a Professional Statement of Need and a Housing Focused Person Centered Plan. We will assist in connecting you with the individuals who can complete these form for you.
All forms are submitted to DHS for approval.
